Attention:  Due to circumstances beyond the control of LifeSpan, Inc. we are forced to change the distribution site from the Sharon Presbyterian Church on Carnot Road to:

The Carriage House Senior Center
494 Thorn Run Road
Moon Township, Pa.
From 9:00 a.m. until 1:00 p.m.

I have attached the flyer and general distribution for all LSI Centers and for Moon. 

We will post signage at the Sharon Presbyterian Church directing individuals to the Carriage House for their FMNP Vouchers.  Please contact me at (412) 464-1300 ext. # 127 if you have any questions or need any further information.
